You can type directly into the forms from your computer and then print them.

: Please note that while you will be able to save this PDF document onto your computer, Acrobat Reader will not allow you to save any text you add to the PDF. We apologize for any inconvenience, but these are the current limits of the technology.

Applicants with Adobe Acrobat 5.0 installed on their computers will be able to save text they add to PDF documents. This software is available from Adobe.

What is Adobe PDF?
With over 200 million copies of Acrobat Reader distributed worldwide, Adobe PDF is the open de facto standard for electronic document distribution. Adobe PDF is a universal file format that preserves all of the fonts, formatting, colors, and graphics of any source document, regardless of the application and platform used to create it. Adobe PDF files are compact and can be shared, viewed, navigated, and printed exactly as intended by anyone with the free Acrobat Reader. Opening and Saving PDF Files

In order to open a PDF file, you will need the FREE Adobe Reader (4.0 or higher) installed on your computer. It can be downloaded from www.adobe.com/products/acrobat/readstep.html. A few clicks of your mouse and the program is installed on your computer.
Once Adobe Acrobat Reader is installed, you can open files stored in PDF format in several ways.

* Double click the PDF file link: Matching Incentive Program Guidelines and Application

OR

* Open Adobe Acrobat Reader first by double clicking the AcroRd32.exe file located on your hard drive, then double click the PDF file and it should open.

OR

* Save the PDF file to a folder on your hard drive. Remember where you saved the file.
Open Adobe Acrobat Reader and then click File/Open (located on the task bar). Find the PDF file that you saved, click once to highlight it and then click open.

PLEASE NOTE: Acrobat Reader will not allow you to save any text you add to a PDF document. Once a form is filled out, print several copies or photocopy a set for your records prior to mailing the signed original.

Filling Out PDF Files

To fill out a form, place the cursor in the area that you want to type in and click. You can use the tab key to move between fields or simply use your cursor and click in the desired area.
In a multiline text field, the information you enter will automatically wrap around once you deselect that field (either by hitting the Tab key or clicking your cursor into another field).
For Best Results:

1. Print out a blank copy of each form first. Use these blank forms as templates to gather all the pieces of data you'll need when completing them on the computer.

2. Type your answers to the longer questions into word processing software first (like Word or Works).

3. Fill out any budget details using a pen and your blank copy of the printed forms. Do your math calculations BEFORE entering budget figures into the forms.

4.
When all your pieces of data for at least one form are ready to be put together, cut and paste any narrative responses into the form and edit for space, and/or key in the budget figures you've assembled. Allow yourself enough time to fill out an entire page of the application in one sitting.

5. When a form is complete, print it. Check that the form prints the way you want it to look BEFORE you close the file.

Printing PDF Files

When you are ready to
print, be sure to use the Print Icon located on the same line/bar as the ADOBE logo and not the Print Icon on your browser's task bar.
